Admissions Information
Required Documents for Admission
The following items are required for admission into Washington School District:
- Copy of Birth Certificate
- Copy of Social Security Card
- Immunization Records
- Proof of Residency – 3 documents are required
Examples:
- Current Real Estate Bill (homeowners)
- Lease Agreement (renters)
- Current paid Per Capita Tax Receipt
- Driver’s License with Current Address
- Bank Statement with Current Address
- Utility Bills (gas, electric, cable, phone) with Current Address
- Letter from Social Security Office with Current Address
- Letter from Public Assistance Office with Current Address
To expedite the enrollment process, the following items are also needed:
- A copy of the student’s up-to-date transcript
- Current schedule
- Immunizations record
